VNA Hospice Care accepts referrals from family members, friends, doctors, discharge planners, primary caregivers, home health aids, social workers, clergy and spiritual counselors.

*Referral to hospice early in the diagnosis of the life-limiting illness means getting the most out of the full range of services available and ensuring the most supportive program of care for the patient and their family.
