VNA Hospice Care is dedicated to enhancing quality of life and respecting the patient’s and family’s right to make end of life decisions that emphasize dignity and pain and symptom management. The patient’s primary care physician directs an individualized plan of care coordinated by the VNA Hospice Care Team. Care is provided in the patient’s home or in any “home-like” setting.

Comprehensive Services Include:
- Skilled Nursing visits: 24-hour-a-day, on-call availability of registered nurses with expertise in pain and symptom management
- Personal care assistance
- Advocacy in accessing community resources based on the needs of the patient and family
- Spiritual support and pastoral counseling provided by our chaplain
- Companionship and respite for caregivers offered by trained volunteers
- Home medical equipment
- All pharmaceuticals as related to the primary diagnosis
- Nutritional counseling
- Specialty Programs for patients diagnosed with:
- Alzheimer’s/Dementia
- Cancer
- Cardio-Pulmonary Disease
- Renal or Liver Disease
- HIV
- Heart Disease
- Long-Term Care Program for patients in extended facilities (allows patients in facilities to receive the same access to hospice care available to patients in their homes)
- Bereavement Support following the loss of a loved one for adults and children. Telephone support, various structured support groups and drop-in sessions.
